Negotiations
This information is for
an Employer and Union who are already participating in the Plan.
From time to time, through collective bargaining, the Employer and
Union may agree to an increase in the benefits they wish to provide
to Plan Participants. The Supplement Form should be used to
implement such an improvement in pension benefits.
Part One of the
Supplement Form consists of Definitions and Instructions. Part
Two – Supplement Specifications is where the Employer and Union
complete all of the items according to the Collective Bargaining
Agreement. Both parties should sign the form and send it,
along with a copy of the Collective Bargaining Agreement, to the
address on the last page of the form.
